 We
work with many insurances and have contracts with some for
their allowed amount. In the
event that your insurance company does not work with us, we
have a sliding fee scale policy that goes by gross income
of a household. An application must be filled out and on file
before the sliding fee scale can be used. A copy of last years
tax return, front and back page is required. The Executive
Director reviews the application to determine if you qualify
for the sliding fee.
